Guildcard # is 42112628 Slot # is 2 I think (it's the 2nd character i made so i hope this is correct) I didn't get pics of before and after sorry wasn't really thinking about that.

So i was playing on my hunter ChibiTsuna and the game crashed as i was equiping an item to the character. When i logged back on all of the items i had equipped to my character were gone and not in my inventory. the rest of my items were ok in my inventory. This happened on 2/13/2014 around 6pm eastern time.
